Register for online access – under 14s

Use this service to register a child under the age of 14 to use GP online services.

You can use this service if you:

• are registered at the surgery

Before you start

Please ensure as parent/guardian you already have an online account of your own before completing an application for your child.

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
• 2 forms of photographic identification
• 1 recent proof of address in the parent/carer’s name

Once online access has been set up and you have activated the account you will have full access to your child’s medical record via proxy access. Full access will remain in place until their 14th birthday when it will be switched off. Access will then cease and the patient will need to apply for online access themselves by downloading the NHS App.
