Wellbeing Matters Social Prescribing at St Andrews Medical Centre

Would you like to find out more on what support is available to you to help with your health and wellbeing goals?

Social prescribing is a way of connecting people to non-medical support and resources within their community to improve their health and wellbeing. St Andrews Medical Centre has a dedicated Community Connector who can support you with non-medical, lifestyle and social needs. This could be finding out what support is out there in your community and choose from a menu of options tailored to you. This could be connecting to social and creative activities, engage in healthy lifestyle behaviours such as physical activity and moving more, connecting to communities of identity and experience, learning a new skill or prepare for employment at your pace, volunteering and much more to improve your health and wellbeing.

Please speak to your GP reception team for a referral to Wellbeing Matters Social Prescribing.

Wellbeing Matters Social Prescribing WorkWell

Are you off work and want support with your health and options to get back to work? We have support through Wellbeing Matters Social Prescribing with a WorkWell Community Connector.

What is WorkWell?

WorkWell is a programme that supports disabled people and people with health conditions to start, stay or succeed in work. WorkWell can offer a range of support, including:

  • a personal assessment of your needs to understand what support might work best for your circumstances
  • personalised, goal-based plans to address your physical, mental wellbeing, and social needs to help you return to and/or thrive in work
  • access and referral to local and community-based work and health support services, including:
    • musculoskeletal (MSK) assessments and treatment support for conditions which affect all areas of the body including the spine, arms, legs, hands and feet
    • support to access non-medical wellbeing support, services and activities, for example volunteering, arts activities, group learning, gardening, befriending, cookery, healthy eating advice and a range of sports
    • impartial careers advice, information, guidance and support to access jobs, skills and training, including help to progress in work
  • support for employers to understand your needs and advice on how to provide workplace adjustments that support you (employers will only be contacted with your permission)

Each individual participating in WorkWell will receive tailored support in the form of one-to-one appointments. These will be with a dedicated WorkWell Community Connector to review progress and recommend further actions and activities related to the personalised action plan.
